New Patients
Initial Appointment
Your initial appointment will consist of an exam and evaluation explaining your diagnosis and treatment options. Occasionally, surgery can be performed the same day as the exam. However, a complex medical history or treatment plan will require an evaluation and a second appointment for the procedure in order to provide the safest and best patient care possible. For patients undergoing IV anesthesia for surgery, we typically will see the patient for a preoperative consultation and examination by our doctors before the surgery date. This consultation also allows the patient to meet with our financial and insurance coordinator to discuss and answer any questions.
Please assist us by providing the following information at the time of your initial visit:
- Your surgical referral slip if given to you by your dentist or specialist and any x-rays if applicable.
- A list of medications you are presently taking.
- A list of medical allergies.
- Your primary physicians name and phone number.
- If you have medical or dental insurance, bring any issued cards.
- A photo ID

IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or legal guardian at the initial visit and procedure appointment.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, osteoporosis, etc.).
X-Rays
If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. Additional x-rays may be necessary for our specific diagnosis, and can be taken at our office.
